What is Fistula

First of all we will discuss about Fistula. Fistula is an abnormal connection between two vessels or organs. Fistula can occur anywhere in the body but mainly it occurs in vagina, intestine, skin, and rectum.

Difference between Colovesical fistula and Anal Fistula

1. Colovesical fistula is an abnormal connection between the colon (large intestine) and urinary bladder while in Anal fistula there is an abnormal connection between the anal canal and peri-anal skin. It’s a small opening seen in the anal canal which discharges pus.

2. Colovesical fistula is an uncommon condition as compare to the anal fistula.

3. Colovesical fistula occurs due to diverticulitis, colorectal cancer, as a complication of colon radiotherapy or crohn's disease  but anal fistula mainly occurs due to blockage or clogging of anal glands, due to chronic constipation and obesity.

4. Colovesical fistula can occur at any age but Anal fistula mainly occurs at the age of 20-40.

Colovesical fistula

Colovesical fistula is a very painful condition, in this condition the fecal matter enters from colon to bladder which causes severe pain and other complications.

Causes - Colovesical fistula can occur due to the following reasons

Diverticulitis or Diverticular disease In Diverticulitis, inflammation occurs in the diverticulum, which are small pouches formed along the large intestine wall.

Colorectal Cancer

As a complication of Surgery  of Bladder or Colon and Radiotherapy, - Crohn's disease

Clinical features - It can be present with the following signs and symptoms

  1. Diarrhea and Abdominal pain
  2. Pneumaturia - It is known to be the most common symptom. In this bubbles are seen in the urine which  is formed due to the mixing of gas from Colon with the urine.
  3. Haematuria stands for blood in the urine. It can be of two types
  4. Gross haematuria - In which traces of blood are visible  in the urine.
  5. Microscopic haematuria - In which blood in the urine is not visible with the naked eyes but only detectable microscopically
  6. Fecaluria - It is the condition in which fecal matter is seen in the urine. The color of the urine becomes brownish and it becomes a little cloudy.
  7. Dysuria - Dysuria stands for a condition in which burning sensation occurs during urination or urine passing.

Diagnosis - Colovesical fistula can be diagnosed by following diagnostic methods

  • Physical examination
  • Urine examination
  • Cystography
  • X-ray and Scans
