Abstract

Wegener's granulomatosis (granulomatosis with polyangiitis) is a disease characterized by damage to small vessels, with the formation of specific granulomas, in various organs and systems of the human body. In this article, we are going to discuss what Granulomatous with Polyangiitis is, and how we can manage this disease with the help of Ayurveda. With proper care and treatment, one can live a happy and comfy life for a prolonged time.

What is Granulomatous with polyangiitis?

Granulomatous with polyangiitis formerly known as GPA or Wegener's Granulomatosis.  It is a systemic granulomatous-necrotizing vasculitis of small-caliber vessels, with a predominant lesion of the upper respiratory tract (URT), lungs and kidneys, associated with antineutrophilic cytoplasmic antibodies (ANCA). GPA is considered to be a relatively rare disease.

What is ANCA?

ANCA has associated vasculitis (AAV) is a group of systemic autoimmune diseases characterized by inflammatory necrotizing lesions of the walls, predominantly small (capillaries and venules) and, much less often, middle vessels of various organs, as well as the frequent presence of antineutrophilic cytoplasmic autoantibodies (ANCA) in the blood serum.

Primary AAV includes:-

  • Granulomatosis with Polyangiitis (GPA).
  • Microscopic Polyangiitis (MPA).
  • Eosinophilic Granulomatosis with Polyangiitis (EGPA).
  • ANCA-associated vasculitis with isolated kidney damage.

Causes of Granulomatous with Polyangiitis

The cause of Granulomatous with Polyangiitis (GPA) is not known. It is not a contagious disease. It doesn’t occur within families.

Symptoms of Granulomatous with polyangiitis

The classic picture of Wegener's granulomatosis includes the following symptoms:

  • Pain in the musculoskeletal system, Joint Pain.
  • General Weakness, headache, fever.
  • Necrotic and dystrophic processes in the organs of granulomatosis.
  • Hoarseness, cough.
  • frequent nosebleeds;
  • Increased sweating at night;
  • Numbness of the hands;
  • Diseases of systems and organs are characteristic (pleurisy, renal failure, liver cirrhosis).
  • Loss of appetite.
  • Dizziness.
  • Blood in the urine.

Differentiating indicators, which clearly indicate the presence of Wegener's granulomatosis, is damage to the upper respiratory tract, lungs, and kidneys.

There are 4 stages of development of Wegener's granulomatosis

Stage I

Rhinogenic Granulomatosis

Stage II

Pulmonary - accordingly, necrotic-ulcerative lesions of the lungs are involved in the process of pathogenesis, secondary inflammation is formed.

Stage III

Generalized lesion –A direct generalization of the pathological process, diseases of the kidneys, blood vessels, organ of vision, etc.

Stage IV

Terminal - the development of respiratory and renal failure, which is not compatible with human life.

There is also a limited form of the disease, without kidney damage. However, such cases occur in 15-20%. If the kidneys are involved in the process, then noticeable changes in the body immediately occur hematuria (blood in the urine), back pain, proteinuria (a significant amount of protein in the urine).

Unfortunately, at the moment, mortality due to Wegener's granulomatosis is quite high, which is due to the late diagnosis of the disease and the subsequent complete destruction of the respiratory system and kidneys.

Complications in Granulomatous with polyangiitis

  • Deafness.
  • Development of necrosis throughout the body.
  • Destruction of the bones of the hands, adrenal insufficient.

Therefore, you need to take care of yourself, treat each disease from beginning to end. The period of remission in patients is no more than six months.
Granulomatosis can be provoked by many processes in the body:

  • Taking hormonal drugs.
  • Viral infections (influenza and measles, scarlet fever, adenoviruses and herpes viruses, Epstein-Barr).
  • Ionizing radiation.
  • The intake of chemicals (benzene) and immunosuppressant can also provoke this disease.

Who is more prone to Granulomatous with polyangiitis?

The disease can affect all organs, but the most common targets are the upper and lower respiratory tract, as well as the kidneys. GPA usually develops in middle-aged and older people (40–50 years), equally in men and women. The disease often develops in girls during adolescence. The average age of patients at the time of the onset of the disease does not exceed 40 years.

Diagnostics for Granulomatous with polyangiitis

Methods for diagnosing granulomatosis include the processes of searching for antigens, on their own cells, as well as factors of rheumatism (C - reactive protein).

Instrumental methods are used to locate foci and apply local therapy. This therapy uses the X-ray method to examine and search for parts or foci that are affected by granulomatosis.
